EMI Health Dental Providers |
Below is a brief description of the three dental plan options to choose from:
Value Discount Plan – This discount plan is free to USEA members. The plan can also be used to cover additional family members for a small annual fee. You and your family can now receive quality care at reduced prices, saving you up to 70% on many dental services. This is a discount plan, not an insured plan. You must receive care from a participated provider (over 700 providers). Please see the USEA Dental Plans Brochure for more detailed information on this plan. USEA members must complete the enrollment form for their free coverage.
Advantage Plan – This plan will give you the ease of knowing what your out-of-pocket expenses are before going to the dentist. You will receive quality care, excellent benefits with low co-payments. This plan also allows you to receive care in or out of network. But we recommend you to stay in the network to maximize your benefit (over 1,100 providers). Please see the USEA Dental Plans Brochure for more detailed information on this plan.
Choice Plan – The Choice Plan replaces our Premier Plan. This new Choice Plan enhances the benefits of the Premier Plan by adding the Advantage (Plus) Network as an option. Enhanced benefits are accessed by using a provider that is on the Advantage Plus Network. By utilizing the Advantage Plus Network, there is no deductible and a significantly higher annual maximum limit. This plan is an incentive PPO plan with higher in-network benefit levels. Choice give you the flexibility to receive care from any provider you choose, either in or out of the network (over 1,600 providers). Please note: Monthly premiums will be payroll deducted (Except Value Plan). The actual monthly premium amount deducted depends on the number of payroll deductions your district may allow for the year. The monthly premiums indicated above are based on 12 deductions per year. Coverage for the 2012-2013 year will begin September 1, 2012 and will run through August 31, 2013. If you are currently enrolled in one of our USEA dental plans and wish to continue, you do not need to fill our an enrollment form. However, if you choose to switch from one plan to another, or make any change in dependant coverage, please fill out a new enrollment form and return it to USEA.
If you enroll your spouse or dependents in the Value Plan, you must pay the yearly premium in advance. We do not have a payroll deduction option for the Value Plan.
